RN Community Liaison

Mercy is seeking an RN Community Liaison to build strong relationships between our healthcare teams, patients, families, and community partners. In this role, you will coordinate transitions of care, provide education on services and resources, and promote Mercy's programs across the community. You'll collaborate with physicians, care managers, and external agencies to improve access, reduce readmissions, and support wellness initiatives. Responsibilities

• Serve as liaison between Mercy, patients, families, and community partners to support access to services
• Coordinate safe transitions of care from hospital to home or other settings
• Educate patients and community organizations about Mercy programs and available resources
• Build and maintain referral relationships with physicians, clinics, and community agencies
• Participate in community health events and outreach initiatives to promote wellness
• Collaborate with interdisciplinary teams to reduce readmissions and improve patient outcomes
• Document patient interactions and outreach activities in the EMR accurately and timely
• Identify service gaps and provide feedback to leadership on community needs
• Support development and implementation of community-based care initiatives
• Uphold Mercy's culture of compassion, diversity, and respect in all interactions
