LPN, LPN​/LVN, Healthcare Nursing

Encompass Health is seeking a Licensed Practical Nurse to deliver high-quality, patient-centered care in our inpatient rehabilitation hospital. The LPN provides direct nursing care, including medication administration, treatments, and monitoring, while supporting patients recovering from major illnesses and injuries. Working in a collaborative, interdisciplinary team, you'll help implement care plans, reinforce therapy goals, educate patients and families, and ensure safety and adherence to clinical and regulatory standards, all within a culture focused on integrity, respect, and professional growth.

Responsibilities

• Provide direct patient care including vitals, wound care, medication administration, and ADL support under RN/physician direction
• Assist with rehabilitation therapies, monitor patient progress, and document care accurately in the medical record
• Collaborate with an interdisciplinary team to implement individualized care plans and meet rehabilitation goals
• Educate patients and families on treatment plans, medications, and safe discharge practices
• Maintain patient safety, infection control, and compliance with regulatory and Encompass Health standards
