RN Home Health PRN

Seeking compassionate Home Health Registered Nurses (RNs) to provide skilled care in patients' homes across multiple counties.

• Assessing patient conditions and coordinating individualized care plans.
• Performing tasks such as vitals, wound care, IV therapy, blood draws, and injections.
• Collaborating with physicians and multidisciplinary teams to update and revise care plans.
• Documenting visits, treatments, and patient progress accurately and timely.
• Engaging with patients, families, and caregivers to ensure quality care and address concerns.
• Managing discharge planning and ensuring compliance with policies and regulations.

• Graduate of an accredited nursing program; no prior experience required.
• Valid RN license in applicable states (Illinois, Missouri, Oklahoma, Wisconsin) with appropriate certifications such as BLS.
• Valid driver’s license and proof of insurance for travel.
• Physical ability to perform clinical tasks and frequent driving.
• Strong communication, assessment, and documentation skills.
