Location: St. Louis
We are seeking a dedicated Registered Nurse (RN) Home Health Case Manager to provide patient-centered care in North Kansas City, MO.
Key Responsibilities Include
- Managing a personal caseload with supervision of LPN visits, scheduling, and delivering a variety of care such as routine visits, evaluations, and care starts
- Educating patients on health conditions like CHF, COPD, Diabetes, Wound Care, and Medication Management
- Collaborating with physicians to obtain orders and develop diagnosis-driven plans of care
- Working closely with therapy teams including PT, OT, and Speech
- Active Missouri or Compact RN License and valid Missouri Driver's License
- Strong clinical assessment and diagnostic interpretation skills
- Organized, self-directed, compassionate communicator with patients, families, and physicians
- Home health experience preferred
