Rehab - Clinical Nurse; Part-Time

Role Overview:

Provides direct patient care, including assessment, diagnosis, planning, implementation, and evaluation within nursing care standards. Promotes patient and family-centered care by educating on illness management, post-treatment care, and medication administration. Participates in safety, quality, and regulatory activities, and engages in professional development.

• Develops, implements, and documents individualized care plans with collaborative input from patients and the interprofessional team.
• Assesses physical, psychosocial, and emotional needs, considering patient preferences, values, and cultural factors.
• Ensures effective communication of care plans across the continuum of care.
• Works in a safety-sensitive environment, maintaining alertness and safe practices at all times.

• Education:
  
  Nursing Diploma/Associate's Degree required;
  Bachelor's Degree preferred.
• Licenses &
  
  Certifications:
  
  Valid RN license.
• Experience:
  
  No prior experience required; 2+ years preferred.
• Strong assessment, critical thinking, communication, and teamwork skills.

This role involves working in a 24-hour environment, collaborating with PT, OT, and Speech therapists, and supporting a patient-centered care approach.
