Case Manager – Street Outreach Program

We provide case management and advocacy for runaway, homeless, and at‑risk youth, collaborating with partners to reduce barriers and promote self‑advocacy.

• Provide case management and advocacy services for at‑risk youth.
• Collaborate with internal and external partners to reduce barriers and facilitate access to community resources.
• Support youth in developing self‑advocacy skills.

• Bachelor’s degree in Social Work, Psychology, Sociology, or related field OR high school diploma with at least four years of human services experience OR comparable combination.
• Experience working with at‑risk youth and group facilitation.
• CPR certification required.
• Valid Class E driver’s license.
