In this role, you will act as a liaison between clients, providers, and community resources to ensure coordinated, person-centered care. You will help identify client needs, facilitate service connections, and support collaboration across multidisciplinary teams to improve outcomes.
Key Responsibilities- Serve as a liaison between clients, providers, and community resources to ensure coordinated care
- Engage individuals and assess needs related to behavioral health, social services, and community support
- Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports
- Collaborate with interdisciplinary teams to develop and support individualized service plans
- Advocate on behalf of clients to reduce barriers to care and improve access to services
- Build and maintain relationships with community partners and service providers
- Monitor client progress and follow up to ensure continuity of care
- Maintain accurate and timely documentation in accordance with organizational and regulatory standards
- Participate in team meetings, case reviews, and program development activities
- Support outreach efforts and community engagement initiatives
- Promote a recovery‑focused, trauma‑informed, and culturally responsive approach to services
- Perform other duties as assigned by leadership
- Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred
- Relevant experience in behavioral health, case management, or community services may be considered in lieu of education
- Knowledge of community resources and behavioral health systems preferred
- Must have a valid driver’s license with an acceptable driving record and current auto insurance
- Strong communication and relationship‑building skills
- Ability to work effectively with diverse populations and maintain confidentiality
- Strong organizational and time‑management skills
- Ability to multi‑task and respond effectively in a fast‑paced environment
- Ability to collaborate with interdisciplinary teams and community partners
- Ability to exert up to 10 pounds of force occasionally
- Ability to perform repetitive hand, finger, and arm movements for typing and documentation
- Ability to sit for extended periods with occasional walking or standing
- Ability to meet the physical demands of a sedentary work environment
