Position: Community Behavioral Health Liaison I
Location: St. Louis

Job Summary

Join our compassionate and collaborative team as a Community Behavioral Health Liaison, where you will play a vital role in connecting individuals to critical behavioral health and community-based services. This position offers the opportunity to make a meaningful impact by supporting individuals in accessing care, navigating systems, and achieving stability and improved wellbeing. In this role, you will act as a liaison between clients, providers, and community resources to ensure coordinated, person-centered care. You will help identify client needs, facilitate service connections, and support collaboration across multidisciplinary teams to improve outcomes.

Key Responsibilities
  • Serve as a liaison between clients, providers, and community resources to ensure coordinated care
  • Engage individuals and assess needs related to behavioral health, social services, and community support
  • Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports
  • Collaborate with interdisciplinary teams to develop and support individualized service plans
  • Advocate on behalf of clients to reduce barriers to care and improve access to services
  • Build and maintain relationships with community partners and service providers
  • Monitor client progress and follow up to ensure continuity of care
  • Maintain accurate and timely documentation in accordance with organizational and regulatory standards
  • Participate in team meetings, case reviews, and program development activities
  • Support outreach efforts and community engagement initiatives
  • Promote a recovery‑focused, trauma‑informed, and culturally responsive approach to services
  • Perform other duties as assigned by leadership
Education, Experience, and/or Credential Qualifications
  • Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred
  • Relevant experience in behavioral health, case management, or community services may be considered in lieu of education
  • Knowledge of community resources and behavioral health systems preferred
Additional Qualifications
  • Must have a valid driver’s license with an acceptable driving record and current auto insurance
  • Strong communication and relationship‑building skills
  • Ability to work effectively with diverse populations and maintain confidentiality
  • Strong organizational and time‑management skills
  • Ability to multi‑task and respond effectively in a fast‑paced environment
  • Ability to collaborate with interdisciplinary teams and community partners
Physical Requirements
  • Ability to exert up to 10 pounds of force occasionally
  • Ability to perform repetitive hand, finger, and arm movements for typing and documentation
  • Ability to sit for extended periods with occasional walking or standing
  • Ability to meet the physical demands of a sedentary work environment
