Working ConditionsTypical office setting, home, or community location as agreed; complete paperwork and documentation. Meet clients in home, foster home, residential facility, hospital, or other locations as needed. Prioritized driver with reliable transportation; routine driving, may lift up to 25 pounds. Requires manual dexterity, normal hearing and vision, and ability to visit more than one family per day.
Position SummaryThe Foster Care Case Manager (FCCM) is responsible for the safety and services of children and families in foster care. Tasks include facilitating Family Support Team meetings, developing individualized care plans, coordinating services across settings, coordinating providers in home, school, and community, and documenting support activities. The FCCM promotes effective utilization of support services to achieve permanency for children and families.
EssentialJob Functions
- Follow all contractual and agency policies, procedures, and standards for service delivery.
- Maintain a caseload of 12 to 15 children, adjusted for program capacity and agency needs.
- Provide full case management services to children and families in the Missouri foster and adoptive care system from assignment until case closure or transfer.
- Document all activities and maintain data entry in FACES and CSWIS accurately and timely.
- Have face‑to‑face contact with each assigned child and family at least monthly; maintain telephone contact with child and family support providers regularly.
- Evaluate and report the impact of services to team members and the judiciary; attend all court hearings; schedule, coordinate, and facilitate FST meetings within prescribed time frames.
- Participate in the FCCM consortium on‑call schedule as employee on‑call (EOC) for PCHAS on a rotating basis.
Candidate must possess a bachelor’s degree in social work, psychology, counseling, education, criminal justice, or a related human services field. Master’s degree preferred. Minimum of two (2) years of relevant case management experience.
Skills- Represent PCHAS to the professional community with strong verbal and written communication; build and maintain positive community relationships with personnel, placement and treatment providers, schools, and agency partners.
- Build rapport with children, parents, and families; actively manage and resolve conflicts. Demonstrate excellent time management, organization, planning, and assertive communication skills.
- Work a flexible schedule as approved by the FCCM Supervisor, respond to crises, and maintain confidentiality and HIPAA standards.
- Operate computers and agency software, copier, calculator, telephone; navigate multiple data entry systems.
- Analyze complex variables and select effective interventions; handle novel and diverse problems, resolve conflicts, make decisions under pressure, maintain a long attention span, and work with people from diverse cultural backgrounds.
- Valid driver’s license and risk‑free driving record.
- Undergo child abuse/neglect, criminal history, sexual offender, and childcare/elder care disqualification list screenings conducted by the Missouri Department of Health and Senior Services and the Family Care Safety Registry.
- Submit fingerprints for review by the Missouri State Highway Patrol and the FBI.
- Undergo a pre‑employment drug screen and pre‑employment physical; results must show no evidence of communicable disease.
- Maintain American Red Cross first aid and CPR certification at all times.

Salary Information$22.15 – $25.50 hourly wage
