Join our compassionate and collaborative team as a Community Behavioral Health Liaison, where you will play a vital role in connecting individuals to critical behavioral health and community-based services. You will help identify client needs, facilitate service connections, and support collaboration across multidisciplinary teams to improve outcomes.

Key Responsibilities:
Serve as a liaison between clients, providers, and community resources to ensure coordinated care Engage individuals and assess needs related to behavioral health, social services, and community support
Facilitate referrals and connect clients to appropriate services including medical, mental health, housing, and social supports
Collaborate with interdisciplinary teams to develop and support individualized service plans
Advocate on behalf of clients to reduce barriers to care and improve access to services
Build and maintain relationships with community partners and service providers
Monitor client progress and follow up to ensure continuity of care Maintain accurate and timely documentation in accordance with organizational and regulatory standards
Participate in team meetings, case reviews, and program development activities
Support outreach efforts and community engagement initiatives
Promote a recovery‑focused, trauma‑informed, and culturally responsive approach to services
Perform other duties as assigned by leadership
Education, Experience, and/or Credential
Qualifications:
Bachelor’s degree in social work, psychology, counseling, or a related human services field preferred
Relevant experience in behavioral health, case management, or community services may be considered in lieu of education
Knowledge of community resources and behavioral health systems preferred
Additional Qualifications:
Must have a valid driver’s license with an acceptable driving record and current auto insurance
Strong communication and relationship‑building skills
Ability to work effectively with diverse populations and maintain confidentiality
Strong organizational and time‑management skills
Ability to multi‑task and respond effectively in a fast‑paced environment
Ability to collaborate with interdisciplinary teams and community partners
Physical Requirements Ability to exert up to 10 pounds of force occasionally
Ability to perform repetitive hand, finger, and arm movements for typing and documentation
Ability to sit for extended periods with occasional walking or standing
Ability to meet the physical demands of a sedentary work environment
