Job OverviewThis position is responsible for providing casework to program clients. The core position focus is assisting clients with reentry while recording client progress and maintaining documentation.
Responsibilities and Duties- Under supervision, this job requires support and structure to clients developing program plans by managing a caseload of clients, and administering the policies and procedures of Dismas House, Programs Manager, and BOP.
- Documents program services and ensures that records are completed according to established policies and procedures.
- Conducts individual or group sessions as needed for program orientation and job readiness training as needed.
- Manages and implements approved daily and weekly accountability plans for the clients.
- Advises clients on personal, family, financial, employment, crisis management, health, substance abuse and education matters.
- Maintains and manages client files with all required agency paperwork and completes all paperwork in a timely manner.
- Coordinates client requests for permission to drive, obtain cell phones, relocation and furloughs.
- Coordinates release planning with the Program Review Team (PRT).
- Prepares release plans, release papers, terminal reports, and related documents.
- Monitors client issues and advises supervisor of those issues.
- Coordinates client Individualized Program Plan (IPP) with the social service coordinator, client, client’s family and other agencies as appropriate.
- Recommends remedial action or sanctions in response to noncompliance.
- Manages client status in the electronic monitoring program (EMP).
- Ensures accurate and correct schedules and inclusion zone information is current in the EMP’s software.
- Reviews the EMP’s dashboard for caseload at least once per shift and responds accordingly.
- Conducts and documents weekly timeline review with clients in the EMP.
- The required education, experience and qualifications for Case Manager is a four-year degree in a social, human services, corrections, or community-based services. At a minimum, one year of experience must be working in human services, corrections, or community-based services. Work experience may be substituted for academic studies by exchanging one year of work experience in a related field for one year of academic education.
Total work experience needed in lieu of the combination of education and work experience is five years. - Proven ability to effectively communicate, manage time and multi-task in the effort to serve our clients.
- Requires the ability to work with individuals with criminal, asocial, and addictive personality types.
- Must be knowledgeable of community resources, the local employment market, available education, training, and vocational resources or be able to develop such knowledge
- Ability to work with computers and the applicable software typically used in this line of work
- Physically able to reach, stretch, bend, and walk during daily routine, with the ability to stand for long lengths of time
- Valid driver's license and appropriate liability insurance. Positions in this classification may be required to travel and work at multiple sites on a regular basis
- First Aid / CPR / AED certification completed within probationary period and maintained throughout employment
