Part Time St. Louis Area Case Manager

St. Patrick Center provides opportunities for self-sufficiency and dignity to people who are homeless or at risk of becoming homeless. Individuals and families build permanent, positive change in their lives through safe and affordable housing, sound mental and physical health, and employment and financial stability. Working collaboratively with individuals, businesses, government and other service providers, St. Patrick Center will end chronic homelessness in the St.

Louis region through Housing First, an evidence-based practice that tells us to remove barriers, do intake and assessment early, move people into housing and offer them support to achieve their goals.

The St. Patrick Center, a ministry of Catholic Charities of The Archdiocese of St. Louis, is hiring a part time St. Louis Area Case Manager, who will use best practices to assess the housing needs of veterans who are experiencing homelessness or at risk of becoming unhoused that are enrolled in the Support Services for Veteran Families (SSVF) program. The case manager will utilize a wrap-around philosophy to assist veteran families in maintaining self-sufficiency and housing stability by meeting with veteran families to develop an Individual Service Strategy/Treatment Plan of Care, coordinating individualized services for housing and financial stability, and documenting all support related to activities and services.

This position works 20-25 hours per week and starts at $20/hour.

Job Responsibilities
  • Assess client program eligibility at initial intake
  • Assess the client's strengths, desires, and immediate/long term needs
  • Assist with stably housing clients within 30 days of enrollment
  • Make in-home visits to observe indicators of the client's progress
  • Assist clients with their transportation, clothing, furniture and food needs
  • Assist clients with monthly budgeting
  • Contribute to the ideas and processes of the ICM team
  • Collaborate with SSVF Peer Support services as needed
  • Receive input from ICM team members regarding housing, mentoring, substance use, and other immediate/long term needs
  • Responsible for meeting goal indicators as outlined in the SSVF grant
  • Assist and complete special case management projects as directed by program Manager or program Director
  • Complete daily and monthly departmental paperwork
  • Update clients' electronic files in Case Worthy daily, to include communication with clients, services provided, or any internal or external referrals made
  • Create and maintain an organized hard copy file for each client
  • Refer clients to additional resources to address other service needs such as substance abuse, mental health and legal assistance
  • Make referrals to other SPC Programs as appropriate
  • Refer clients to local VA medical centers for potential health and financial benefits assistance.
  • Other duties as assigned.
Job Requirements

College degree required BSW or related field preferred. Prefer 1-2 years' experience working with the unhoused population preferred. Knowledge of housing and other supportive service resources Driver's license (MO residents require a Class

E) Current and valid auto insurance Reliable vehicle
