Position Description

The Program Coordinator will oversee and manage St. Lazare House, ensuring that clients receive the appropriate support, resources, and services to help them achieve long-term stability. Responsibilities include client outreach, eligibility screening, counseling, and coordinating housing and services in compliance with HUD guidelines. The Program Coordinator will manage documentation, reports, and ensure clients are connected to resources and maintain progress toward permanent housing.

Additionally, the position includes supervising staff and collaborating with service providers to support client needs, while reporting to and working closely with the St. Louis City Director.

Primary Duties And Responsibilities Essential Functions
  • Set policy and programming to ensure that clients are getting the support that they need and that all guidelines are followed as outlined by Grantors and other funders
  • Outreach/screen potential clients for eligibility using the Coordinated Entry Front Door for all referrals
  • Welcome new clients, educate them on program guidelines, and provide counseling to assist with lease understanding, utility setup, and movingin process.
  • Mediate conflicts or issues between tenants and/or between tenants and staff in a constructive and empathetic manner, fostering a safe and respectful environment for all participants.
  • Ensure ongoing compliance with HUD guidelines for PSH rental assistance programs
  • Ensure timely submission of required reports and documentation to remain in grant compliance (e.g., monthly activity and finance reports, Annual Progress Report, grant renewals).
  • Ensure client charts include required eligibility documents, progress notes, treatment plans, and other necessary paperwork
  • Work with maintenance to fix identified problems prior to clients movein and while clients reside in units
  • Conduct a psychosocial and needs assessment with each client at entry
  • Develop with clients at entry and yearly thereafter an individualized housing and service plan, including planning a path to permanent housing
  • Provide individual and group counseling to help clients improve quality of life, support clients in addressing behaviors and building skills for stable housing and develop healthier coping strategies
  • Provide medication education and management
  • Assist clients in obtaining and maintaining eligible benefits, seeking job training, improving job skills, pursuing education (e.g., GED), and finding employment
  • Connect clients to community resources and supports and encourage ongoing participation
  • Coordinate care with other service providers and involved supports
