JOB TITLE: Site Coordinator

REPORTS TO: Training and Transition Program Coordinator
JOB PURPOSE: Provide On-site support for vocational trainees to ensure job skill attainment and retention. Analyze job requirements and environmental conditions of the client's workplace to develop a person's employment plan for successful integration to the new job, to include instruction in work skills and social skills. Use various teaching techniques, strategies and ADA standards to provide appropriate job accommodation. Assist in the application process, including the development of interview skills, resume, building, and the identification of appropriate employment goals.
JOBRESPONSIBILITIES Overview
- Coordinate and perform client intakes, vocational training, and Vocational Rehabilitation (VR) services as needed for students in the Vocational Skills program.
- Manage support caseloads of 10- 20 students
- Work with stakeholders, students, and families to develop Person Centered Support Plans that include vocational and career goals.
- Develop key relationships to promote program training and employment opportunities in the community.
- Provide direct support to students along the continuum of supports from the classroom to employment or additional training.
- Work under the direction of the Rise program coordinator as well as on‑site program leadership.
- Provide on‑site vocational training supports to participants in the following areas:
- Assist with skills training and incorporation of strategies as needed (e.g. task analysis, pictorial tools) to assist learning and retention of required job tasks.
- Support the retention and instruction of appropriate social skills
- Assist with the development of a transition plan to adult services
- Collaborate closely with worksite leadership to encourage employment and training opportunities
- Collaborate with on‑site program staff on the completion of Task Analysis data
- Attend participant IEP meetings and parent teacher conferences where appropriate.
- Provide case management to participants graduating the program, this can include job development services, vocational training, counseling, independent living, or other community resources.
- Recommend and coordinate adaptive equipment and technology needed.
- Provide education to employers regarding the employment and accommodations participants may require for them to be successfully employed.
- Negotiate work related issues such as schedules, job modifications, benefits, etc., with employers and other persons who have significant roles in the participants’ lives.
- Provide clients transportation to potential employment sites, jobs fairs, and interviews
- Maintain a record of all jobs applied to and follow up actions
- Provide participants' with completing on‑line applications.
- Assist with job shadowing and or job trials as necessary
- Communicate regularly with Vocational Rehabilitation Counselor regarding job development and client progress
- Market and develop community relationships to promote employment and inclusion of individuals with disabilities in the community.
- Complete daily support documentation, notes, mileage, and hours in Set Works
- Assure the timely completion of all necessary billing documents:
