Job Overview

Fairview is seeking a Family Medicine Physician to join our primary care team at M Health Fairview Clinic – Midway
. This is an excellent opportunity to join a thriving practice with multiple on‑site specialties and a long-tenured, highly experienced support staff.

Practice Details:

• 12 provider primary care practice with more than 7 specialists onsite.
• Patient encounters average 18-22 patients per day.
• 34 patient contact hours per week.
• Call Frequency: 1:18 low intensity - phone consultation only with first tier nurse triage.
• As part of Fairview’s Accountable Care Organization (ACO), providers practice in a collaborative, team‑based model focused on quality outcomes, care coordination, and population health management.
• Openings: 0. 8 to 1.0 FTE (Full Time) - Flexible schedule.
• Fully integrated Electronic Medical Record (EPIC).

• Graduate of an accredited ACGME/AOA Family Medicine residency training program.
• Board Certified/Board Prepared with the American Board of Medical Specialties or American Osteopathic Association.
• Licensed or ability to obtain Minnesota Board of Medical Practice Licensure.
• Ability to obtain and /or maintain DEA certification for the State of Minnesota.
• Meet threshold criteria/qualifications for Credentialing and Privileges.

The posted pay range is for a 1.0 FTE. The actual rate of pay offered within this range may depend on several factors, such as FTE, skills, knowledge, relevant education, experience, and market conditions. Additionally, our organization values pay equity and considers the internal equity of our team when making any offer. Positions with the same min/max are flat rate jobs based on 1.0

FTE schedule. Compensation plans based on production can allow for higher pay than the range posted.
