This position exists to provide professional support to the Division of Health Equity Strategy and Innovation (HESI) grant management, project management and engagement work. The position is responsible for the implementation, coordination, evaluation and planning of the Eliminating Health Disparities Initiative and other grant programs as provided by the state legislature and other funding sources. This position is responsible for providing technical assistance to grantees, including those that may have limited experience in applying for and implementing a state funded grant program.
The position is responsible for co-developing innovative strategies to enhance grantee performance and build their capacity to understand ways to advance health equity and share the learning from the EHDI and other grant programs to a broader set of interest holders, policymakers, MDH leadership and program staff, health care systems, community members, etc.

* Primary Responsibilities:
*
* + Manage and implement EHDI's state grants to local communities, including launching and providing Request for Proposal (RFP) support, budgeting, site visit support, and invoice reviews.
+ Provide consultation, guidance, technical assistance and support to local community grantees so they can successfully implement effective initiatives to reduce health disparities in the communities most impacted, including but not limited to communities of color, American Indians, immigrants and refugees, and LGBTQ and other marginalized communities.
+ Provide cross-functional collaboration with other offices and divisions across the agency to advance efforts aimed at reducing disparities in chronic health conditions and improving outcomes in infant mortality initiatives.
+ Evaluate and monitor the impact of the Eliminating Health Disparities Initiative grantee activities.
+ Assist with strategic planning, event-planning, report writing, presentation and other assistance for the Eliminating Health Disparities Initiative Grant Program and other grant programs, including establishing goals for the designated program(s).
+ Support the development and management of budgetary component for the Eliminating Health Disparities and other grant programs, including monitoring expenditures, providing financial reconciliation support, and calculating projections.
+ Perform duties as requested for the Division of Health Equity Strategy and Innovation and MDH.

Candidates residing outside of Minnesota further than 50 miles of the assigned work location at the time of application must be able to relocate to Minnesota within 30 days of their start date.
