Summary

The role of the Adult MH‑TCM is to help adults with serious and persistent mental illness with severe emotional disturbance gain access to medical, social, educational, vocational and other necessary services connected to the person's mental health needs. Targeted case management services include developing a functional assessment, an individual community support plan for an adult and an individual family community support plan.

It also includes referring and linking the person to mental health and other services while ensuring coordination and monitoring of the delivery of services.

The Adult MH‑TCM will support anti‑oppressive/anti‑racist practice, and trauma‑responsive practice in all aspects of their interactions with staff, community, and company stakeholders through their administrative and operations support of program staff working on site and in community to address socioeconomic and health disparity. Comfort and proven experience and interest in working with staff with a range of identities including age, developmental and acquired disabilities, religion, spirituality, ethnicity, race, socio‑economic status, sexual orientation, relationship structure, indigenous heritage, national origin, immigration status, and gender is required.

Essential Duties and Responsibilities
  • Meet 28‑30 face‑to‑face contacts per month, and fill extra time coordinating care for clients who are on civil commitment
  • Coordinate integrated services and support necessary to assist each client as they work towards their goal of recovery.
  • Partner with clients to develop an Individual Community Support Plan that identifies services and supports that will enable each client to live as independently in the community as possible or to develop an Individual Family Community Support Plan that identifies services and supports needed by the client and their family to improve the well‑being of the child and improve family functioning.
  • To the extent requested by the client, provide collaboration and education to significant others and family members who are interested in assisting the client in their recovery process
  • Treat all clients with dignity and respect, providing services that are respectful of diagnosis, age, gender, and culture
  • Assure that services are implemented as developed by the client, family and team as documented in the Individual Community Support Plan or Individual Family Community Support Plan, and that services are continuously monitored and evaluated.
  • Assure that all changes in client status or needs are communicated through documentation.
  • Participate in staff meetings and trainings
  • Other related duties may be assigned
Qualifications/Experience
  • Mental health practitioner or hold a bachelor's degree in social work, psychology or human services related field from an accredited college or university and meets requirements for supervision and continuing education
  • At least 2000 hours or more of supervised experience in the delivery of mental health services to adult
  • A case manager without 2,000 hours of supervised experience in the delivery of services to adults with mental illness must:
  • Receive clinical supervision regarding individual service delivery from a mental health professional at least one hour per week until the requirement of 2,000 hours of experience is met; and
