Overview
Bright Path LLC is seeking outgoing, compassionate, and organized Mental Health Specialists to support individuals in our Unit-Based Services program. In this role, you will help people build essential life skills, promote independence, and ensure services are delivered with dignity, respect, and full compliance with Minnesota’s 245D licensing standards, in collaboration with people’s support team.

As a Unit-Based Services Mental Health Specialist at Bright Path, you will provide person-centered, hands-on support to individuals with disabilities, older adults, and those with mental health support needs in community-based settings. You’ll work directly with people served to build adaptive skills, increase independence, and create safe, supportive environments. The work is dynamic, meaningful, and best suited for someone who thrives in both independent and team-based settings, is adaptable to changing program needs, and is committed to delivering services with dignity and respect.
This mobile role requires regular travel across the Twin Cities metro area to meet with clients and provide training and shadow shifts for new employees.
Key Responsibilities- Provide Direct Support:
Help individuals with physical disabilities, mental illness, and developmental disabilities gain independence by building adaptive skills and supporting daily living needs in community settings - Develop Emergency Plans:
Assist individuals in creating personalized 24-Hour Emergency Back-Up Plans to ensure safety and preparedness - Deliver Mobile Services:
Travel throughout the Twin Cities metro area to meet with clients, provide direct hands-on support, and assist the Designated Coordinator with essential tasks - Train and Support Staff:
Provide shadow shifts and training for new employees to ensure consistent, high-quality service delivery - Ensure Compliance:
Document progress, report incidents, and maintain adherence to Minnesota’s 245D regulatory guidelines - Collaborate Across Teams:
Work closely with waivered service providers, mental health teams, social workers, and other professionals to coordinate care - Crisis Prevention and Response:
Play a key role in preventing crises and ensuring appropriate interventions during emergencies - Participate in On-Call Rotation:
Provide 24-Hour Emergency Assistance support on a rotating schedule
- Compassionate, empathetic, and patient, with strong problem-solving skills
- Strong organizational and time-management skills
- Valid driver’s license and access to a reliable, insured vehicle (mileage reimbursement provided)
- Ability to pass criminal/registry background checks in accordance with Minnesota DHS guidelines
- At minimum, a 2-year degree in Human Services or related field and 2 years of direct experience working with individuals with Intellectual/Developmental Disabilities or related conditions
