SUMMARY

The Clinical Psychologist/Therapist provides clinical services and evidence based/best treatment to assigned caseload including assessment, diagnosis, treatment planning, therapy, skills development. This clinician will provide individual therapy. The Clinical Psychologist/Therapist will communicate and collaborate with collateral contacts by consulting with providers regarding integrated health, current function, medication related- issues, educations support and any other care related to client/families. This position also assesses patient safety and risk, development of safety plans, reporting of any abuse or neglect, and coordinating hospitalization or any other emergency situations as needed.

ESSENTIAL

FUNCTIONS
  • Make assessments and interpretations based on biopsychosocial evaluations, psychometric tests, semi-structured interviews, therapy sessions, and cognitive assessments.
  • Collaborate with other healthcare providers to evaluate the medical and physical condition of clients.
  • Formulate and create personalized interventions, treatments, and management plans for clients.
  • Assess clients' risk, considering their mental health and social needs when performing interventions.
  • Interact with clients and other appropriate parties regarding interventions and progress.
  • Employing various treatment methods such as psychotherapy, cognitive behavioral therapy, mindfulness, behavior modification, and stress reduction therapy.
  • Measure the effectiveness of interventions and implementing changes where required.
  • Ensure the maintenance and safe storage of accurate program and client records, and all other pertinent documents.
  • Maintain confidentiality of clients’ treatment and records in accordance with HIPAA. Participating in clinical training seminars and mentoring assistant psychologists, psychology students, and interns on-site.
  • Assist primary health care providers in recognizing and treating mental disorders and psychosocial problems.
  • Work with primary care team to treat and manage patients with chronic emotional and/or health problems and substance use concerns efficiently and effectively.
  • Assist in the detection of "at risk" patients and development of plans to prevent further psychological or physical deterioration.
  • Completes psychological and cognitive testing for select patients (such as the MoCA)
  • Attend and participate in staff meetings and Quality Improvement activities as required.
  • Employees must work effectively on a team and communicate respectfully with patients and all staff based on shared goals and mutual respect. This includes demonstrating adaptability to continuous efforts to improve patient care and willingness to be an active care team member engaged in ongoing process and quality improvement projects within the framework of the Patient-Centered Medical Home.
  • Honors patients’ right to privacy and confidentiality. Adheres to strict patient, customer and business confidentiality standards.
  • Complete timely, accurate, and compliant documentation in the EMR, including but not limited to assessments, diagnoses, treatment plans, progress notes, risk/safety assessments, safety plans, coordination of care, collateral communications, and other updates reflecting medical necessity.
  • Perform other tasks as assigned.
