Position: Primary Care Medical Provider (MD,PA,NP)

Axis Clinics is a Federally Qualified Healthcare Center Look Alike (FQHC-LAL) and Essential Community Provider (ECP) serving patients across three locations in Minneapolis. Role Description

This is a full-time, on-site Primary Care Medical Provider role located in Minneapolis MN. Responsibilities include delivering primary medical care to patients, diagnosing and treating acute and chronic illnesses, developing care plans, collaborating with other healthcare team members, and educating patients on disease prevention and health maintenance. The provider will be expected to contribute to quality improvement initiatives and provide compassionate support for diverse patient populations.

Qualifications
  • Strong skills in Medicine and the ability to diagnose and treat various conditions effectively. Primary Care Focus, experience, or training is preferred.
  • Excellent Care Communication skills to build trust and interact with diverse patient populations.
  • Proficiency in Training and educating patients on health maintenance and disease prevention.
  • Current licensure as an MD, PA, or NP in Minnesota with a focus on high-quality care delivery.
  • Commitment to practicing patient-centered care and addressing the needs of underserved communities.
  • Ability to work collaboratively with a multidisciplinary team.
  • Bilingual or multilingual skills are considered a plus (Somali, Oromo, Amharic, Spanish)
