Physical Therapist Assistant – Skilled Nursing

• Assist patients/residents/clients with therapy activities and exercises under the supervision of a physical therapist
• Provide, monitor, and record therapy services for patients/residents/clients with physical, cognitive, and developmental impairments
• Select, implement, and modify therapeutic activities and interventions consistent with competency levels, patient goals, and practice-setting or state-licensure requirements
• Exchange information with and provide documentation to the physical therapist about patient responses and communications during intervention
• Provide information and documentation related to achievement of targeted physical therapy outcomes
• Follow accepted clinical practice guidelines of the relevant professional organization
• Perform other related duties as requested
• Based on facility needs and assigned locations, provide intervention in post-acute, long-term care, and home health care environments

• Appropriate education level required in accordance with state licensure
• Graduate of an approved American Physical Therapy Association (APTA) program as a Physical Therapist Assistant
• Licensed as a Physical Therapist Assistant in the state(s) of practice
• Conformity to supervision and licensing standards of the state(s) where services are provided
• Basic Life Support (BLS) certification required within six months of employment
• Valid driver's license when applicable

Demonstrates expertise in providing therapy services under the supervision of a physical therapist, with a focus on patient-centered interventions and adherence to clinical practice guidelines. Proficient in documentation and communication regarding patient progress and therapy outcomes.
