Salary Range: $19.00 To $21.00 Hourly
About the Role:

Together with Wilder’s Healthy Aging & Caregiving Services, you will join a collaborative team that supports the health and independence of older adults as they age and receive care in the community, which is critical to Wilder’s mission of improving lives today and for generations to come.
As a valued team member, you will provide person-centered care to older adults and adults with chronic conditions, supporting them with daily living assistance, including housekeeping, laundry, medication administration, and personal care. You will promote client safety, implement care plans, and provide basic rehabilitative services utilizing rehabilitative principles as instructed by the site nurse. Communicate regularly with an interdisciplinary team to support client care and complete documentation in accordance with program policies and procedures, regulatory requirements and best practice standards.
Qualifications:
High School Diploma or equivalent required. Prior experience working in a health care setting with older adults and/or disabled adults preferred. Certified Nursing Assistant, Home Health Aide Certificate or Trained Medication Assistant required.
