Salary

LPN: $28-35/hour

RN: $36-43/hour

Position Summary

This position will work in collaboration with triage nursing to make skilled nursing visits to patients/families needing after‑hours/overnight support. Friday. Saturday and Sunday shifts available.

  • Qualifications
    • Graduate of accredited school of nursing
    • Current state of Minnesota RN or LPN licensure
    • Three years recent nursing experience in acute, long‑term care or home care/hospice/public health
    • Patient care planning and patient teaching skills
    • Written and verbal communication skills
    • Critical thinker
    • Current Minnesota driver’s license, insurance and daily access to reliable vehicle
    • Computer literacy
    Job Duties
    • Displays an attitude of courtesy and respect for all prospective patients, families, staff and referral sources.
    • Works closely with triage nurse and makes face‑to‑face visits as directed.
    • Functions as a team member with other staff in the performance of duties.
    • Communicates significant findings, problems, patients’ needs, changes in condition or environment to the physician, clinical manager and other members of the hospice care team and obtains physician orders as required.
    • Seeks and accepts feedback regarding the performance of job duties.
    • Is aware of patient confidentiality and adheres to HIPAA regulations.
    • Displays an understanding of vulnerable adult issues.
    • Displays an understanding of hospice patients’ rights.
    • Exhibits sound mature judgment.
    • Excellent patient care delivery.
