Job Summary

The Hospice Registered Nurse Case Manager (RNCM) is responsible for delivering comprehensive, patient-centered nursing care to hospice patients as appropriate to the plan of care. The RNCM coordinates and manages the interdisciplinary plan of care, ensuring comfort, dignity, and quality of life for patients and families. This role serves as a key liaison within the communities and facilities they support, fostering strong collaborative relationships to promote continuity of care, effective communication, and positive hospice outcomes.

Duties/Responsibilities
  • Assume primary responsibility for patient and family needs by assessing, planning, implementing, and evaluating the plan of care.
  • Demonstrate competency in completing the initial comprehensive assessment upon hospice admission.
  • Maintain ongoing communication with providers and interdisciplinary team members to coordinate optimal care and appropriate use of resources.
  • Provide education to patients, families, and facilities regarding medications, including dosing, administration, and potential side effects.
  • Assess psychological, social, and spiritual needs of the patient and family and collaborate with the Social Worker and/or Spiritual Care Coordinator as appropriate.
  • Coordinate and communicate medication orders and changes with the Nurse Practitioner and Hospice Physician.
  • Coordinate timely delivery of medications, durable medical equipment, and supplies for patients and caregivers.
  • Communicate equipment and supply needs with the Director of Nursing.
  • Collaborate with the Director of Nursing, Hospice Physician and Nurse Practitioner to determine and adjust the appropriate level of care based on ongoing nursing assessment.
  • Document all on-call activities, including phone calls and visits, in the electronic health record (EHR) within 24 hours.
  • Document all routine and PRN visits, phone calls, and patient interactions in the EHR within 72 hours.
  • Evaluate and update the patient Plan of Care and identified problems at least every 14 days or as condition changes.
  • Provide appropriate guidance and support at the time of death, including postmortem care, medication disposal witnessing, and required notifications (e.g., coroner, funeral home).
  • Implement nursing interventions that support outcomes of a comfortable, dignified, and safe death.
  • Conduct monthly supervisory visits with LPNs and Hospice Aides in accordance with regulatory requirements.
  • Maintain communication with facilities and caregivers through the hospice binder, calendar updates, and visit documentation at each encounter.
  • Demonstrate safe and effective use of physical and mechanical transfer techniques when assisting patients.
  • Accurately stage wounds, obtain appropriate treatment orders, and communicate the care plan to families, caregivers, and facility staff.
  • Build and maintain positive community relationships with referral sources, facilities, and community partners to support care coordination, promote hospice services, and enhance continuity of patient care
Required Skills/Abilities
  • Knowledge of appropriate HIPAA and confidentiality
  • Able to work as both a part of a team and independently
  • Possesses great organizational Skills
  • Excellent written and verbal communication skills
  • Valid driver’s license and auto insurance
  • Ability to use EHR systems and operate mobile devices with efficiency
