Nurse Practitioner​/Physician Assistant - Primary Care; Group Home Population

Bluestone Physician Services provides care to patients with complex, chronic conditions and disabilities.

We are hiring Nurse Practitioners (NP) or Physician Assistants (PA) to provide team-based care in group homes across the Minneapolis/St. Paul area.

• Provide on-site, team-oriented primary care to patients within group home settings.
• Part-time providers care for patients weekdays; schedule tailored to part-time preferences.
• Manage a panel of patients autonomously, seeing approximately 8-14 patients per day.
• Ability to design and manage your schedule.
• Develop and implement care plans appropriate to patients and their desires.
• Receive clinical support from clinical assistants and care coordination through LPNs, RNs, and social workers.
• Collaborate with community partners within home care, hospice, and other resources as needed.

• Graduate of an NP or PA program from an accredited school.
• Must have and maintain a valid Minnesota license.
• Professional clinical experience in primary care.
• Proven ability to communicate effectively with strong verbal skills.
• Excellent interpersonal and customer-service skills.
• Must have a valid driver’s license and minimum state-required liability auto insurance; local travel is required.
• Demonstrated compatibility with Bluestone's mission and practicing philosophies.
• Travel throughout St. Paul and surrounding communities is required.

• 2+ years of clinical experience and/or group home experience in another role.

• Compensation Base + Quality/Productivity: $115,000–$140,000 USD.
