Position Overview

Nurse Practitioner (NP) or Physician Assistant (PA) – Primary Care, Group Home Population. St. Paul, MN. Bluestone Physician Services delivers great outcomes by bringing exceptional care to patients living with complex, chronic conditions and disabilities. Our multidisciplinary care teams collaborate with patients, their families and other healthcare providers to deliver care that is preventative, proactive and tailored to their unique needs.

Using an evidence-based approach focused on quality care management and data-driven medical decisions, Bluestone care teams collaborate to manage patients’ chronic conditions, address social determinants of health, manage transitions to and from inpatient settings, provide behavioral health support and more. Under our model of care,
Bluestone patients experience 21% fewer ER visits, 36% fewer hospitalizations and 41% fewer hospital readmissions compared to patients with similar conditions and complexities over the same time period. Our care teams travel directly to patients who reside in Assisted Living, Memory Care and Group Home communities throughout Minnesota, Wisconsin and Florida and are supported by clinical operations and administrative colleagues who work remotely or at our corporate offices in Stillwater, Minnesota, and Tampa, Florida.

Join our growing practice as a Nurse Practitioner (NP) or Physician Assistant (PA) and bring exceptional team-based care directly to patients residing in Group Homes in the northeast metro of Minneapolis/St. Paul.

• Provide on-site, team-oriented primary care to patients within the group home setting living communities
• Part-time providers care for patients on weekdays; tailored to part-time desires and needs
• Manage a panel of patients autonomously, seeing approximately 8-14 patients per day
• Ability to design and manage your schedule
• Develop and implement care plans appropriate to patients and their desires
• Clinical support from clinical assistants and care coordination through LPNs, RNs and social workers
• Collaborate with community partners within home care, hospice, and other resources as needed

• Graduate of NP program or PA program from an accredited school
• Must have and maintain a valid Minnesota License
• Professional clinical experience in primary care
• Proven ability to communicate effectively with strong verbal skills
• Excellent interpersonal and customer service skills
• Must have a valid driver’s license and minimum state required liability auto insurance; local travel is required
• Demonstrated compatibility with Bluestone’s mission and practicing philosophies
• Travel throughout St. Paul and surrounding communities required

• 2+ years of clinical experience and/or group home experience in another role

• Compensation Base + Quality/Productivity: $115,000 - $140,000 USD
