Job Overview

Ebenezer Integrated Care & Rehab (EICR) serves seniors with long‑term, transitional, and dementia care needs in downtown St. Paul. As part of Fairview’s Community Health and Wellness Hub, EICR delivers innovative, collaborative care. The Hub, opened in 2022, offers a variety of services—from primary and mental health care to enrichment options for seniors, food access programs, and community gathering spaces. It partners with local organizations such as Minnesota Community Care and Sanneh Foundation to make essential services easier to access.

We are seeking a Nursing Assistant (CNA/NAR) for our new Skilled Nursing Community (Transitional Care units, Long‑Term Care) in downtown St. Paul, Minnesota. This role is responsible for providing resident care and services.

Schedule
  • 40 hours every two weeks;
    Part Time
  • Day Shift; 6:00 am – 2:30 pm
  • Block

    Schedule:
    • Week 1:
      Thursday and Friday
    • Week 2:
      Wednesday, Thursday, and Sunday
  • Transitional Care Unit
  • Every other weekend and holiday rotation
Responsibilities
  • Assist residents with personal cares based on their plan of care:
    • Provide AM/PM cares
    • Provide full bath/shower weekly
    • Provide oral hygiene per care plan
    • Provide nail care, shaving as appropriate
    • Check hearing aids; clean eyeglasses daily
    • Ensure residents are clean, dry, and dressed appropriately
    • Provide offloading for residents
    • Perform Restorative Nursing Programs per facility policy
    • Provide adequate nutrition and hydration (distribute meal trays, assist residents as needed, document intake/output meal consumption, provide nourishment and water between meals as ordered)
    • Ensure individual toileting plan is followed
    • Answer call lights promptly
  • Observe residents and report status to appropriate staff:
    • Receive report at the start of the shift
    • Give report to team leader at the end of the shift & PRN
    • Notify appropriate staff of any changes in a timely & appropriate manner (i.e., incidents, skin integrity, behavior changes, illness)
    • Offer pertinent observations & input for documentation and care plan updates
    • Use Nursing Assistant Kardex or Care sheet
    • Take and document weights as ordered
    • Document resident cares promptly in EHR
    • Respond to emergency situations appropriately
    • Handle resident and family complaints tactfully and refer concerns to appropriate staff members
    • Follow Ebenezer policies and procedures
    • Report resident change of condition to Licensed Nurse
    • Attend mandatory, annual, and any other educational opportunities on site
  • Perform environmental and other responsibilities as assigned:
    • Maintain order & cleanliness in resident rooms
    • Maintain order & cleanliness in other work areas
    • Report malfunctioning equipment to Maintenance Department
    • Assist with admissions, transfers & discharges as needed
    • Answer phones with proper customer service etiquette
    • Perform other duties as assigned and directed
Required Qualifications
  • Less than 1 year experience
  • Registered Nursing Assistant on the MDH Nursing Assistant Registry by start date – OR an individual hired into Ebenezer’s Nursing Assistant Course or Pre‑CNA job; will receive a 4‑month post‑hire to become active on the Minnesota Nursing Assistant Registry.
Preferred Qualifications
  • High School Diploma or GED
  • 1 year experience
