Licensed Practical Nurse (LPN) - Long-Term Care - Part-Time Days (0.6 FTE)

Job Location:

St. Paul, MN 55104. Position Type:
Part-Time. Salary Range: $30.49 - $37.20 hourly.

Job Shift: Day - 6:30 a.m.

- 3:00 p.m.

Wage & Schedule

Wage: $30.49 - $37.20 per hour.

Schedule:

Part-Time Days - 0.6 FTE (48 hours per pay period).

Hours:

6:30 a.m.

- 3:00 p.m.

What You’ll Do
  • Administer medications and treatments according to physician orders and facility protocols.
  • Assess residents' physical, mental, and emotional status and report changes promptly.
  • Assist in developing and implementing individualized care plans.
  • Supervise and support nursing assistants and other caregiving staff.
  • Educate residents and families on care plans, medications, and health needs.
  • Collaborate with physicians, therapists, and the interdisciplinary care team.
  • Monitor, document, and communicate changes in resident condition.
  • Maintain timely and accurate documentation in the electronic medical record (EMR).
  • Participate in infection prevention, quality improvement, and regulatory compliance initiatives.
  • Promote a safe, respectful, and resident-centered environment focused on dignity and well-being.
What We’re Looking For
  • Current Licensed Practical Nurse (LPN) license in good standing with the Minnesota Board of Nursing.
  • Current CPR and First Aid certification.
  • Eligible to work in federally funded healthcare programs.
  • Strong clinical assessment, communication, and critical-thinking skills.
  • Ability to prioritize responsibilities and work effectively in a fast-paced environment.
Preferred Qualifications
  • Experience in long-term care, skilled nursing, transitional care, assisted living, or memory care.
  • Experience using electronic medical record (EMR) systems.
  • Knowledge of geriatric nursing and chronic disease management.
