As an RN Visit Nurse
, you ll care for patients where they feel most comfortable while making meaningful clinical decisions that help them recover, remain independent, and avoid unnecessary hospitalizations.

• Serve patients throughout St. Paul and the East Metro
• Provide meaningful 1:1 skilled nursing care
• Independence with the support of experienced clinical leaders
• Locally owned,
  Medicare-certified agency with more than 30 years of success
• Strong interdisciplinary collaboration with therapists, physicians, and case managers
• A culture that values quality patient care over rushing from visit to visit

• Provide skilled home health nursing visits in patients  homes
• Complete comprehensive assessments and ongoing evaluations
• Perform wound care
  , medication management, injections, and disease monitoring
• Develop and update individualized care plans
• Educate patients and caregivers to improve health outcomes
• Coordinate care with physicians and interdisciplinary team members
• Document visits accurately in Homecare Homebase
• Identify changes in patient condition and intervene appropriately

• Active Minnesota Registered Nurse (RN) license
• Experience in home health
  , hospital, rehabilitation, skilled nursing, or acute care
• Strong assessment, critical thinking, and clinical decision-making skills
• Compassionate communicator who enjoys educating patients and families
• Ability to work independently while managing a daily schedule
• Reliable transportation and willingness to travel throughout the St. Paul service area
