- Complete comprehensive assessments and ongoing evaluations
- Perform wound care
, medication management, injections, and disease monitoring - Develop and update individualized care plans
- Educate patients and caregivers to improve health outcomes
- Coordinate care with physicians and interdisciplinary team members
- Document visits accurately in Homecare Homebase
- Identify changes in patient condition and intervene appropriately
- Active Minnesota Registered Nurse (RN) license
- Experience in home health
, hospital, rehabilitation, skilled nursing, or acute care - Strong assessment, critical thinking, and clinical decision-making skills
- Compassionate communicator who enjoys educating patients and families
- Ability to work independently while managing a daily schedule
- Reliable transportation and willingness to travel throughout the St. Paul service area
