Pay Range: $72,000 - $90,000 (salaried exempt), based on experience, qualifications, and location. This position also includes unlimited incentive bonuses.

As a Hospice Liaison, you are the bridge between compassionate care and those who need it most. You'll be the trusted face of Vivie Hospice to hospitals, skilled nursing facilities, physicians, and the broader community—helping individuals and families navigate end-of-life decisions with clarity and care. Your mission: ensure smooth transitions, educate on hospice services, and strengthen relationships that expand access to quality, patient-centered care.
As a Hospice Liaison, you will:
- Build & Nurture Referral Relationships – Develop and maintain strong partnerships with physicians, discharge planners, skilled nursing facilities, and community organizations to support timely and appropriate hospice referrals.
- Educate & Advocate – Promote hospice philosophy through compassionate education to referral sources, community groups, and the general public. Serve as a knowledgeable guide for families considering end-of-life care options.
- Support Admissions & Transitions – Collaborate with clinical teams and intake staff to assess patient eligibility and ensure a smooth, dignified transition into hospice services.
- Lead Community Engagement – Represent Vivie Hospice at community events, professional gatherings, and health fairs. Share resources and build brand awareness.
- Partner with marketing to refine outreach tactics and materials that resonate with the communities we serve.
- Analyze & Report – Monitor referral activity and trends. Provide regular reports and feedback to leadership to support growth and quality outcomes.
- Other Duties as Assigned – The duties and responsibilities listed above are representative of the nature and level of work assigned and are not necessarily all inclusive.
This job also requires:
- Clinical background (RN, LPN, or social work) is preferred but not required.
- 2+ years of experience in hospice, healthcare outreach, or a related field.
- Strong understanding of hospice and palliative care principles.
- Ability to manage time independently, prioritize multiple responsibilities, and respond to evolving needs.
- Valid driver's license and reliable transportation.
- Experience with EMR and CRM systems.
- Familiarity with intake workflows and patient admissions processes.
- Ability to pass state mandated background checks.
- Physical capability to perform all essential job functions.
- Ability to read, write, and speak English to ensure effective communication with team members, clients, and visitors.
