We're seeking an experienced Licensed Practical Nurse (LPN) who is passionate about resident-centered care and wants to practice nursing in an environment where clinical expertise, collaboration, and compassion are truly valued.

Wage: $30.49–$37.20 per hour (DOE)

Schedule:

Part-Time Days | 0.6 FTE (48 hours per pay period)

Hours:

6:30 a.m. – 3:00 p.m.

Unlike a traditional long-term care setting, our Green House Homes feature:

• Small households with consistent resident assignments

• More time to provide personalized, relationship-based care

• 3 years of LPN experience required
• Long-term care, rehabilitation, or senior living experience preferred
• Strong clinical judgment and assessment skills
• A nurse who thrives in a collaborative, relationship-focused environment
• Passion for delivering exceptional, person-centered care

• Provide skilled nursing care tailored to each resident s individual needs.
• Administer medications and treatments in accordance with physician orders.
• Assess residents, identify changes in condition, and communicate with the healthcare team.
• Develop and update individualized care plans.
• Collaborate closely with caregivers, providers, therapists, and families.
• Support a culture where residents are known, respected, and empowered to live with purpose.
