RN Field Care Manager

Humana is seeking an RN Field Care Manager to deliver high-quality, member-focused care in home and community settings. In this role, you will conduct comprehensive clinical assessments, create and manage individualized care plans, and coordinate services across providers to improve outcomes and reduce avoidable hospitalizations.
