Coordinator of Care

Visiting Nurse (RN Coordinator of Care)

The Coordinator of Care is a Registered Professional Nurse who will manage all aspects of patient care related to services provided in the home. This would include Conducting home visits, completion of all required documentation and communication with PCP.

Full time schedule:
Monday-Friday Full Time RN

• Sign on Bonus

• NYS RN License

• Valid driver’s license and reliable transportation.
• Strong ability to work independently with minimal supervision Excellent clinical skills Basic knowledge of office applications and equipment (Outlook, Word, Printing and scanning applications).
