Home Care After Hours On Call Triage Visit RN

We are looking for someone to be available to be On Call outside of normal business hours to take calls and triage patients, but also be available to make a visit if there is a need to our patients in their home setting.

Hours:

Full-time | There will be a block schedule of 7 days on and 7 days off. This position is considered full-time and will be on-call outside of normal business hours, generally from 5pm - 8am, Monday
- Friday and weekends from 8am - 8am.

A few must-haves for this position:

• Licensed to practice as a RN in the state of MN. License is active and in good standing.
• Minimum of twelve (12) months of accumulated experience as a registered nurse within the last five (5) years or, if no experience within the last five (5) years, evidence of current (within the last 12 months) clinical knowledge as deemed appropriate by the DHCS.
• One (1) year of hospice or home care experience preferred.
• Able to travel within geographic area serviced by the office. Valid driver's license, reliable transportation, and proof of auto insurance.
• CPR Certification
