As a Case Manager, you will serve as a central point of coordination, working closely with the ACT team, community agencies, and families; connect clients with vital resources and coordinate all aspects of their treatment process, ensure clients maintain contact with their psychiatrist and follow up to support medication adherence, complete comprehensive functional assessments to inform personalized care plans, build strong, therapeutic relationships with individual clients and their families, develop partnerships with other community service providers to create a robust network of support.
