POSITION TITLE:Individual Placement and Support Specialist – RC FSS

REPORTS TO:Program Manager or Senior Program Manager

POSITION DESCRIPTION/PRIMARY OBJECTIVE: This position is responsible for placement services, working directly with employers, funding sources and job seekers to achieve program placement goals. Effectively manage a designated caseload while meeting all critical performance goals including position performance goals and program contract goals. The specialist will work in the community (65% of the work week) meeting people on their caseload, family members, and engaging with employers at their businesses.

ESSENTIAL FUNCTIONS

Enrollment/ Assessment
  • Conductorassistinconductinggroupandone-on-oneprograminformationaloverviewsandevaluationofpotential clients
  • Assess and evaluate participant’s physical, social, vocational, educational and employment situation to determine strengths and identify barriers through spending time with job seeker in community settings
  • Administer appropriate assessment and screening tools per program requirements/guidelines
Case Management
  • Engages job seeker in community settings (including participant homes, libraries, schools, coffee shops, businesses, worksites, parks, etcetera. based on job seeker preference), and establishes trusting, collaborative relationships directed toward the goal of competitive employment in community job settings with other workers who do not necessarily have disabilities
  • Assists job seekers and workers in obtaining information about their benefits and how entitlements will be affected by employment so that participants can make decisions about employment opportunities
  • Conducts job search activities while developing realistic short and long-term goals directed toward positions that are individualized to the interests and uniqueness of the people on his/her/their caseload, following the principles and procedures of the IPS evidence-based model. This includes career exploration, career laddering, and training and education support
  • Developanappropriateplanwithspecifictimelinesandmeasurablegoals together with individual and support teams Provides individualized job support to assist workers in retaining employment. Following the worker’s preferences, supports may be provided at the workplace and/or in the community
  • Provides support to employers as agreed upon by workers, which may include negotiating job accommodations and job supports by the IPS specialist with the employer.
  • Provides community outreach services as necessary to workers when they appear to disengage from the service. Uses a variety of methods to provide outreach.
  • Participates in weekly meetings with mental health and employment services teams and communicates individually with team members between meetings to coordinate and integrate services
  • Monitorandreportonjob seeker progress
  • Maintain compliance with program contract requirements and program policies
  • Maintain job seeker'sconfidentiality, adhere to Federal Data Privacy laws
  • Maintaincasefilesandcasefiledocumentationinclusiveofbutnotexclusiveofthefollowing(per program requirements):

Community&Employer Development
  • Conducts a minimum of six in-person employer contacts per week in the community. The aim of these visits to businesses is to develop relationships with managers/owners who have hiring responsibilities
  • Recruitemployerstoparticipateinprogramactivitiesandhireprogramparticipantsforinternships,volunteer and/or direct employment
  • Maintaincontactandpositiverelationshipswithreferringagencies.

    Thisincludesmonthlyformalreports,regular recruiting visits and responding to information requests as needed
  • Participateinthedevelopmentofprogramflyersforemployers,referringcounselorsorjobseekers
  • Communicate and document employer contacts and positions developed as they occur
