About the Position

The Disability Hub MN Options Counselor is responsible for answering inquiries and providing options counseling to individuals with disabilities. The ideal candidate will thrive in a fast‑paced, high‑volume environment, with strong communication skills and in-depth knowledge of local, state, and federal benefits and assistance programs for individuals with disabilities. A successful Options Counselor will manage customer interactions according to established standards and assurances.

Key responsibilities include conducting person‑centered discovery to explore the individual’s needs, strengths, values, preferences, resources, and goals, and applying person‑centered principles to provide effective counseling. The role also involves educating clients on available public and private sector resources and facilitating self‑direction, self‑determination, and informed choice. This position requires a combination of technical knowledge and interpersonal skills to ensure consumers receive the best possible support in navigating available resources.

Key Responsibilities
  • Provide confidential, neutral direct service to Disability Hub MN consumers.
  • Advocate for Hub consumers when necessary, including but not limited to contacting county staff, DHS staff, Social Security Administration, and other social service agencies.
  • Demonstrate a high level of detailed knowledge of State and Federal benefits, resources, and counsel on available options.
  • Respond to inquiries sent to DHS via e‑mail or phone.
  • Complete required documentation within the CRM database in a timely and accurate manner, in compliance with HIPAA, the Minnesota Data Practices Act, and the Minnesota Human Rights Act.
  • Enter data into timesheets, mileage logs, and update information in the  database.
  • Attend and complete all required training and education upon hire (approximately 250‑300 hours) and ongoing as assigned, including conferences on State and Federal Programs related to callers’ inquiries.
  • Represent Disability Hub MN through community education, promotions, and outreach; develop and maintain relationships with community partners (e.g., county staff, Senior Linkage Line, Veteran’s Linkage Line, and other statewide partners).
  • Attend Agency and departmental meetings while adhering to Agency policies.
  • Follow Hub call standards in compliance with AIRS standards and DHS contract requirements.
Requirements
  • Proficient with computer, including Word, Excel, PowerPoint, and Outlook. Candidates must be competent in database and internet skills.

Qualifications
  • Must reside in Minnesota.
  • 4‑year degree and two years related experience; or a 2‑year degree and 4 years related experience.
  • Lawfully authorized to work in the United States.
  • Employment is contingent upon successful completion of a criminal background check and education verification.
