Position: Section 811 Tenancy Support Practitioner

Starting Salary Range: $57,437 + Office Benefit Package (Parking/Remote Expenses): $180/Month subject to change

This position provides professional, quality program services to individuals served by the Section 811 Program. The Mental Health Practitioner will provide Housing Support Services to people with serious mental illness and/or substance use disorders enrolled in the program in the Metro area. The primary objective is to provide outreach, housing transition, and short‑term housing sustaining services, with the goal of connecting the person to ongoing services.

Hours: Monday – Friday 8 am – 4 pm or 9 am – 5 pm, 40 hours per week

Location: Onsite at the RADIAS Health downtown Saint Paul office

Duties and Responsibilities
  • Engage in coordinated outreach efforts with RADIAS Health PATH team members to build connections and establish rapport with individuals once approved for Section 811.
  • Utilize Housing Stabilization Services funding for transition services when possible, including providing necessary applications and verifications, assisting with apartment tours, utility setup, lease signing, and move coordination.
  • Purchase and pay items related to establishing the participant in the apartment using Housing Stabilization Moving Expense funds (e.g., rent and utility deposits, first month’s rent, past‑due rents or utilities, essential furnishings, household supplies, and moving expenses).
  • Ensure participation of the person served in the selection of household furnishings and supplies.
  • Provide tenancy supports to HUD Section 811 Project‑Based program participants who are experiencing or have experienced long‑term homelessness and serious mental illness in the seven‑county metro area.
  • Assist the property with required applications and verifications.
  • Provide instruction and monitoring on the service areas indicated on the client’s Housing Focused Person‑Centered Plan and other assessments.
  • Maintain records of progress for each face‑to‑face meeting and telephone contact with the client.
  • Decide on appropriate interventions or assist the client in obtaining and maintaining the highest level of independence possible.
  • Ensure standards are met according to state and county guidelines and submit appropriate reporting to Vulnerable Adult, Child Protection, the Ombudsman's Office, and other agencies as required.
  • Provide post‑transition services to assist the program participant in sustaining housing and living successfully in the community, including making appropriate referrals for long‑term support services such as Targeted Case Management, ARMHS, CADI, ongoing HSS, etc.
  • Work collaboratively with the person served and the Property Manager to solve identified problems.
  • Provide problem mitigation services for 811 participants who are at risk of losing housing.
  • Coordinate and cooperate with other agency and community providers to provide the best possible care, completing all necessary Release of Information documents for communication with other providers.
  • Assist clients in creating a plan to work toward service goals during regular meetings.
  • Schedule appointments to provide requested services according to the person’s support plan.
  • Bill for services by completing documentation as required by program policies and procedures.
  • Ensure that persons served have access to information about tenant rights and responsibilities and provide ongoing guidance to all in relation to adhering to lease terms.
  • Transport clients in the community as needed.
  • Attend weekly meetings with DHS, AEON, staff meetings, and other scheduled meetings.

Qualifications
  • Bachelor’s degree in behavioral science or related field.
  • 2,000 hours of supervised experience delivering clinical services to adults with mental illness, or 6,000 hours of supervised experience delivering clinical services in the treatment of mental illness.
Preferred Qualifications
  • Bachelor’s degree in behavioral sciences or related fields.
  • Experience providing outreach services to persons experiencing homelessness and knowledge of services/resources in Ramsey County.
