Purpose
The Floating Case Manager/Housing Navigator provides flexible, trauma-informed, and client-centered case management and housing navigation services to individuals and families. This position supports a diverse range of clients, including single adults, youth, and families, across multiple program sites. The Floating Case Manager/Housing Navigator steps in to provide coverage, continuity of care, and addresses social service gaps during staff absences, transitions, or when additional support is needed.
Responsibilities- Provides comprehensive case management and housing navigation services across various departments.
- Tailors individual meetings and social service coordination to promote success and stability through assessing service needs, develops initial goal plans and coordinates services for residents within 90 days of participant entry, and in six months increments thereafter.
- Maintain a caseload that fluctuates depending on program needs; rotate between scattered-site units, supportive housing, and youth/family housing programs.
- Supports potential tenants as they navigate the housing process; including, but not limited to: housing searches, property owner advocacy, apartment showings, rental applications, credit-repair support, expungement processes, move-in, and facilitate tenancy sustaining interventions.
- Assists clients in accessing community resources, accompany clients to initial and/or significant appointments and meetings.
- Maintains up-to-date and organized client files through an electronic records system; including documentation of assessment of service needs, service plans and goals, and case notes documenting case management services.
- Collects and records all required statistical information, accurately reporting data in a timely manner.
- Participates in recovery and client-centered care, assists families and single adults in creating and following harm reduction plans, and resolves crises as appropriate.
- Fosters a positive, engaged participant community.
- Supports the resident community to increase community building skills and community membership skills.
- Assists participants and families as needed to identify and address community concerns and issues.
- Facilitate peer mediation as needed.
- Assists participants to identify opportunities for community integration and developing natural support networks.
- Function as a team member.
- Follows through with own job responsibilities and assignments.
- Engages in constructive problem solving and conflict resolution, assists other team members in doing the same.
- Provides information needed by other team members in a timely and effective manner.
