Safe Harbor Mobile Case Manager:
Meet youth where they are – beyond the doors of Brittany’s Place Youth Shelter.
180 Degrees believes every young person deserves a real path forward — and sometimes that path starts with someone who can connect the dots. As a Mobile Case Manager, you help youth and young adults secure community resources and plan their next steps, working hand-in-hand with Brittany's Place shelter to coordinate services and support smooth transitions in and out of care.
This role also plays a critical part in Minnesota's Safe Harbor network. You'll support the Safe Harbor Regional Navigator by maintaining a caseload of youth who have been sexually exploited or are at risk of exploitation, serving Region 8 (East Metro), which spans Anoka, Chisago, Dakota, Isanti, Ramsey, and Washington counties. Housed at and supported by 180 Degrees at Brittany's Place, the Safe Harbor Regional Navigator program is where your work directly protects some of the most vulnerable youth in our region.
You're not doing this in isolation, you're closely connected to shelter staff, participating in shelter meetings to keep communication open, and building strong community partnerships that expand what's possible for the youth you serve.
What you'll actually do- Help youth and young adults secure community resources and plan their next steps
- Coordinate services connected to Brittany's Place shelter
- Assist youth and Shelter Case Managers with transitions in and out of shelter
- Participate in shelter staff meetings to ensure open communication of needs and services
- Strengthen community partnerships and lead outreach activities
- Support the Safe Harbor Regional Navigator by maintaining a caseload of you impacted by or at risk of sexual exploitation
- Serve Region 8 (East Metro):
Anoka, Chisago, Dakota, Isanti, Ramsey, and Washington counties
- Be part of a mission-driven program at the intersection of shelter services and Minnesota's statewide Safe Harbor initiative
- Do meaningful work protecting youth who have experienced or are at risk of exploitation
- Build strong community partnerships across full metro region
- Make an impact that reaches youth both inside the shelter and out in the community
You care deeply about connecting young people to the resources and safety they need. You're comfortable building relationships across shelter staff, community partners, and a regional network. You understand the urgency and sensitivity of supporting youth impacted by exploitation. And you want work that's real, human, and meaningful — every single day.
This position offices at Brittany's Place.
This is a full-time position, 40 hours per week, onsite at Brittany’s Place. No direct reports; rather a direct hand in helping youth in the community receive support.
Qualifications- Bachelor’s degree in human services, psychology, social services, social work, or a related field and two years of related case management experience; case management experience may be substituted in lieu of education on a year-for-year basis.
- Two years of outreach work required.
- Two years of experience in the nonprofit sector required.
- Valid driver's license and insurance; dependable transportation for traveling to community partners and events; ability to drive Company vehicle without restrictions.
- Four years of outreach work preferred.
- Two years of experience with Safe Harbor preferred.
- Four years of experience in the nonprofit sector preferred.
- Strong foundation in trauma-responsive and youth-centered practice.
