Housing Case Manager

Position

Summary

The Housing Case Manager helps individuals and families obtain and maintain safe, stable, and affordable housing. This role provides case management, housing navigation, advocacy, and resource coordination services that support housing stability and long-term self-sufficiency.

Key Responsibilities Case Management & Housing Support
  • Assess participant needs and develop individualized housing stability plans.
  • Provide ongoing case management, coaching, advocacy, and follow-up.
  • Assist participants with housing searches, applications, move-ins, and housing retention.
  • Connect participants with community resources, financial assistance programs, and supportive services.
  • Educate participants on tenant rights, responsibilities, and housing success strategies.
Partnerships & Community Engagement
  • Build relationships with landlords, housing providers, and community organizations.
  • Coordinate referrals and collaborate with service providers.
  • Represent Model Cities in community meetings and partnerships.
Documentation & Compliance
  • Maintain accurate participant records and case notes.
  • Ensure compliance with program, funding, and regulatory requirements.
  • Track outcomes and utilize HMIS or other case management systems effectively.
Qualifications Required
  • Associate's or Bachelor's degree in Human Services, Social Work, Community Development, or a related field, or an equivalent combination of education and experience.
  • One to three years of experience in case management, housing services, or related human services work.
  • Strong communication, organization, and relationship-building skills.
  • Proficiency with Microsoft Office and data management systems.
  • Valid driver's license and reliable transportation.
Preferred
  • Experience with homelessness prevention, rapid rehousing, or housing stabilization programs.
  • Knowledge of HUD programs, Housing First, Trauma-Informed Care, and HMIS.
  • Bilingual or multilingual abilities.
Compensation & Benefits

Salary Range: $51,000-$54,000 annually. Compensation is based on qualifications, experience, and internal equity.

Work Environment
