Position

Summary

The Housing Case Manager helps individuals and families obtain and maintain safe, stable, and affordable housing. This role provides case management, housing navigation, advocacy, and resource coordination services that support housing stability and long-term self-sufficiency.

The ideal candidate is compassionate, organized, resourceful, and committed to delivering trauma-informed and culturally responsive services.

Key Responsibilities Case Management & Housing Support

Assess participant needs and develop individualized housing stability plans.

Provide ongoing case management, coaching, advocacy, and follow-up.

Assist participants with housing searches, applications, move-ins, and housing retention.

Connect participants with community resources, financial assistance programs, and supportive services.

Educate participants on tenant rights, responsibilities, and housing success…
