Job SummaryThe Registered Nurse is a professional practitioner who assesses, manages, directs, and provides nursing care activities during the patient's hospital stay and coordinates planning with other disciplines, utilizing a patient/customer‑driven approach.
Essential Functions- Performs comprehensive nursing assessment/reassessment.
- Performs age‑appropriate admission or transfer assessment, obtaining input from family/guardian when appropriate.
- Accurately documents findings.
- Performs assessment of post‑op/post‑invasive procedure patients.
- Assesses and documents education and discharge needs of patient and family on admission and throughout hospitalization.
- Provides patient reassessment documenting observations according to plan of care, changes in condition, status, diagnosis, response to care, procedures, and standards of care.
- Establishes, coordinates, and evaluates a plan of care based on assessment data, patient diagnosis, lab data, tests, procedures, physician orders, protocols, and standards of care.
- Identifies short‑ and long‑term goals based on patient care needs.
- Formulates nursing interventions to achieve desired outcomes.
- Incorporates disease‑specific evidence‑based practice into care plan and documentation.
- Provides and documents nursing interventions based on assessed needs, plan of care, and changes in status.
- Collaborates with health team members for coordination of daily care plan for assigned patients.
- Provides patient care, including accurate hand‑off communication reports.
- Administers and documents medications accurately according to policies and procedures.
- Monitors, maintains, and documents accurate IV fluids and parenteral nutrition per policies.
- Completes referrals as indicated.
- Requests consultation for special needs, equipment, or information.
- Provides patient/family education and discharge planning per documentation guidelines.
- Clarifies physician orders as warranted and acknowledges orders in a timely manner.
- Assists physician with procedures/treatments or delegates to Care Partner.
- Documents readback for all telephone/verbal orders; takes orders only in emergencies.
- Recognizes changes in patient condition and takes appropriate actions.
- Involves family/guardian in decision‑making when appropriate.
- Recognizes risks and takes action.
- Incorporates infection control practices in daily care.
- Communicates nursing care and changes in condition.
- Performs ongoing evaluation of effectiveness of care based on assessment data and interventions.
- Evaluates effectiveness of patient/family education.
- Evaluates and modifies plan of care as indicated.
- Recognizes significant changes in clinical parameters and reports immediately to physician.
- Identifies problems, gathers data, suggests solutions, communicates within appropriate authority lines, works toward problem resolution.
- Reports variations from care/treatment following the occurrence reporting policy and procedures.
- Restraint Care:
Initiate/evaluate alternatives prior to application; apply restraints per procedure; monitor patient response; provide specified care during restraints; consult peers for alternatives; document use thoroughly. - Pain Management:
Assess pain presence; incorporate cultural/spiritual beliefs; implement prevention‑focused techniques; involve family in plan; consider massage, repositioning, immobilization, music therapy. - Abuse Assessment:
Recognize abuse criteria; report signs to relevant authorities; take appropriate action to support patient safety.
