Acute Care Medicine manages patients with a full range of clinical issues—from significant blood dyscrasias, advanced endocrine pathology, heart and renal failure, GI conditions, infectious diseases, asthma and a multitude of other conditions. Because of our patient acuity we provide advanced training to bring new nurses to the highest level of functioning. Each unit is led by a clinical manager and lead charge nurse, supported by an advanced practice nurse and a multidisciplinary team including a case manager, social worker, physical therapist, dietitian and pharmacist.

Provides direct patient care activities including assessment, diagnosis, planning, implementation and evaluation within the guidelines of the standards of nursing care.
Responsibilities- Promote patient and family centered care in a healing environment.
- Educate patients and families on how to manage their illness or injury, including post‑treatment home care needs and medication administration.
- Participate in activities that promote patient safety, quality and regulatory compliance.
- Participate in professional development.
- Develop, implement and document individual plans of care with defined goals in collaboration with the interprofessional team and patients, families or caregivers, in accordance with established nursing care standards.
- Proactively plan and ensure communication of the plan of care across the continuum of care.
- Use critical nursing skills to assess and evaluate physical, psychosocial, and emotional needs according to standards of care.
- Assess patient preferences and barriers to involvement in care, including their values, emotional, spiritual, cultural, and population‑specific needs.
- Work in a constant state of alertness and in a safe manner (safety‑sensitive position).
