In-Home Traveler Nurse Practitioner or Physician Assistant

Overview

Signify Health, part of CVS Health®, seeks a full‑time Clinician (Nurse Practitioner or Physician Assistant) to provide in‑home health evaluations across multiple states. The role requires up to 100% travel within a defined coverage area, including overnight stays, car rentals or flights, and holding multiple state licenses.

• Conduct in‑home health evaluations lasting about 45 minutes, including medication and medical history review, physical examination, and, when ordered, diagnostic tests such as diabetic eye exams, spirometry, and other preventive services.
• Use an iPad and an intuitive clinical workflow; no prescription authority, lab ordering or treatment alteration required.
• Travel locally and across licensed states, including overnight stays, car rentals, and flights as required by coverage area; must hold multiple state licenses.
• Conduct virtual visits as needed.
• Build trusted relationships with patients and coordinate care transitions to appropriate providers.

• Licensed Nurse Practitioner or Physician Assistant with active, unrestricted licenses in the coverage area.
• Board certification (required).
• Ability to obtain and maintain multi‑state licenses as required.
• Flexibility to travel across counties within the state and to other business‑required areas.
• Ability to work in diverse home environments, including those with pets, secondhand smoke or potential allergens.

Primary workplace is the private residence of health‑plan members; may involve varied conditions and require safe procedures.

The typical pay range for this role is $78,331.00 – $, based on experience, education, geography and other factors.
