Job Summary
The Nurse Navigator is a leader who supports, mentors, and guides direct patient care providers. They coordinate delivery of care, assess individual patient needs, and make care recommendations based on severity, intensity, and skill level. The Nurse Navigator ensures timely, safe, effective, high‑quality care, collaborates with bedside nurses on care plans, sets professional conduct expectations, and holds care providers accountable to regulatory guidelines.
This role manages inpatient and outpatient populations and facilitates a new care model, supporting quality, safety, financial outcomes, seamless integration, discharge planning, length‑of‑stay and utilization management, and performance metrics.
- Pre‑op education
- Daily rounding while patients are in the hospital
- Patient discharge disposition
- Follow‑up care coordination
- Maintain alignment with mission, values, and Mercy Service Standards

625 S New Ballas Rd, Saint Louis, MO 63141
Shift: Monday – Friday, 8:00 am – 4:30 pm (no weekends or holidays)
Qualifications- Graduate from an approved school of nursing
- Current RN license in the state of practice
- 2–3 years of experience
- BLS (Basic Life Support) certification required
- Ability to push, pull, or lift 50 pounds regularly; prolonged standing and walking; ability to grip, reach, bend, kneel, twist, and squat
- Preferred:
Bachelor of Science in Nursing - Preferred: ACLS (Advanced Cardiac Life Support) certification
