Location: St. Petersburg
Advent Health in Florida is seeking a Registered Nurse Case Manager to support discharge planning and care coordination. You will participate in multidisciplinary rounds, assess discharge needs, and educate patients and families about post-acute options to ensure timely transitions.
You will collaborate with Payors for post-acute care authorizations and leverage community resources to meet social needs. Prior experience in care management is preferred; current RN license is required.
