Position: Licensed Practical Nurse (LPN)
Location: St. Petersburg

SUMMARY AND DUTIES:

Position Summary:

The Licensed Practical Nurse (LPN) is responsible for supporting providers (physicians, nurse practitioners and physician assistants) in the provision of patient care, including but not limited to recording vital signs, discussing chief health concerns, performing testing, and administering injections; assists with procedures and documentation.

Areas of Responsibility:

Resident Health Management/Medication Administration Ensures the residents/participants receive correct medications, correlating doctor’s orders, e-MAR and Medication Card.

Ensures that medications are administered properly and in a timely manner, following doctor’s orders.

Monitors therapeutic positioning, range of motion, and maladaptive behaviors, with any needed interventions.

Observes and records the therapeutic effect of drugs and reports unusual responses to the doctor.

Ensures that drug and biological materials are stored in the appropriate places, at the proper temperatures, internals separate from externals.

Ensures that the Emergency Boxes are maintained according to Pharmacy Best Practices with prompt replacement of any drugs by the nurse who opens the box.

Ensures that any discontinued or outdated drugs are removed from the clinical areas, discontinued on the e-MAR, and states disposition to DON’s Office for destruction of controlled drugs.

Practices universal precautions at all times.

Maintains infection control in accordance with facility and federal guidelines at all times.

Documentation and Recordkeeping Counts and records narcotics and other controlled substances at the beginning and ending of each shift.

Ensures proper nursing documentation and recordkeeping for optimum clinical intervention, including but not limited to:

Accurately completing e-MAR entries.

Accurately transcribing doctor’s orders and Individual Record Notes.

Submits accurate and timely reports for nursing assessments, tracking forms, etc.

Accurately completes Incident Reports; follows-up and provides immediate correction as necessary.

Communication Responds to questions from residents/participants pertaining to IPs.Provides positive attention, interaction and encouragement to residents/participants during the delivery of any nursing service.

Delivers formal/informal services in a timely manner.

Facilitates the scheduling of lab work and promptly reports abnormal results to the doctor.

Promptly communicates with doctors, dentists, pharmacists, DON’s and Day Programs to address any clinical concerns.

Provides accurate information regarding residents to other nurses at shift change.

Provides accurate and timely information regarding issues that may impact residents/participants’ health, safety to relevant departments.

Promptly reports all equipment breakdowns and/or service issues to the DON’s.Positively interacts with the Circle of Support Team to ensure productive outcomes.

Infection Control and Safety Measures Ensures that residents’ personal hygiene is maintained including nail grooming, condition of ears, and oral hygiene.

Ensures that direct care staff practices, good hand washing techniques, wear a moderate nail length and appropriate jewelry;
Reports unsafe practices to the DON’s and the Residential Manager.

Maintains a clean environment in medication rooms, refrigerators, medication carts, suction machines, otoscopes, etc.

Monitors the physical plant for any safety hazards; reports concerns to the DON’s and the Residential Manager.

Observes the infection control measures being implemented by staff (e.g., handling of linen, bed pans, etc.); reports concerns to the DON’s and the Residential Manager.

Utilizes universal precautions when performing clinical task like drawing blood, changing dressings, etc.

EXPERIENCE AND

EDUCATION REQUIREMENTS:

MINIMUM EDUCATION AND WORK EXPERIENCE REQUIREMENTS:

High school diploma or equivalent, required

Current State of Florida License as a LPN; one year experience, preferred.

Previous experience providing services and support to individuals with intellectual and/developmental disabilities in an ICF, Group Home or Adult Day Training program is a plus.
