Keystone Home Health is a CHAP accredited Medicare Certified agency which has been delivering compassionate, high-quality care to patients and their families since 2007. Keystone Home Health is seeking superior caregivers to deliver excellent and effective care in Pinellas and Pasco Counties. We offer competitive pay and benefits package. We are currently looking for an experienced Home Health RN Clinical Manager to join our team.

• The RN Clinical Manager is responsible for coordination and management of patient care and is responsible for the supervision of clinical personnel to ensure that care and services are delivered appropriately.
• Supervises clinical staff and ensures the quality of patient care under federal, state and local regulations and accreditation standards.
• Monitors clinical documentation for completeness and accuracy and ensures it meets standards for good clinical documentation and successful billing.
• Oversees the completion and implementation of the Plan of Care.
• Reviews and approves orders written by field staff and corrects as needed.
• Ensures effective coordination of care with physicians, referral sources, interdisciplinary team, and other individuals as necessary.

QUALIFICATION GUIDELINES EXPERIENCE/TRAINING/

EDUCATION/CERTIFICATION:

• Registered nurse with current licensure to practice nursing in the state
• Possess and maintain CPR certification
• Previous experience in home care setting with one year management or supervisory experience, Required
• Proven ability to work within an interdisciplinary setting
• Must be a licensed driver with an automobile that is insured and in accordance with state and/or organization requirements and is in good working order
• Prior use of Electronic Medical Records (EMR) - Required

• Knowledge of accepted professional standards and practice, Medicare Conditions of Participation, and federal, state, and local regulatory requirements
• Demonstrates excellent observation, nursing judgment, and communication skills
