Community Based Care Coordinator

The Salvation Army Sallie House, St. Petersburg, FL 33713

Schedule/

Hours:

Regular Full-Time at 40 hours/week, Sunday-Thursday 10:30 AM - 7:00 PM ETThis position is responsible for:

Providing case management, care coordination, and scheduling of individual & group counseling for the residents of Sallie House. Maintains case files for all Sallie House residents.

Key responsibilities:

Prepares individual service plans for children following contract guidelines and accreditation standards; utilizes information gathered from direct observation, community input and staff interviews.

Schedules clinical appointments with outside vendors based on the individual needs of the children.

Meets with outside clinical service providers to understand the specific challenges for each resident and use the information to better serve each resident.

Physical Requirements and

Working Conditions:

Work is performed in a normal child care/home environment where there may be physical discomforts associated with changes in weather or discomforts associated with noise, dust, dirt, and the like. Work is often outside in playgrounds, courtyard, parks, beaches, pools.

Work may require driving a vehicle where there may be discomforts associated with heavy traffic or changes in weather.

Must be able to work flexible hours including some evenings and weekends.

Education and Experience:

Master’s degree in social work, counseling or related field and a minimum of two years’ experience in the field of social work, one of which has to have been in child welfare services, OR Bachelor’s degree in social work and a minimum of four years’ experience in the field of social work, two of which have to have been in child welfare services.

Licenses and

Certifications:

Must be cleared by credit checks, Local Law Enforcement, State of Florida Dept of Children and Families, and FBI background screens

Valid State of Florida Driver's License and driving record approved by Territorial Headquarters Risk Management.

Must possess valid CPR/First Aid Certification.
