Registered Nurse; RN - Home Health - per visit

BAYADA Home Health Care is seeking an experienced Registered Nurse (RN) for a full‑time home health care role in Salem, Massachusetts. The position requires OASIS documentation experience and offers a bonus incentive. The RN will work as part of a multidisciplinary team delivering skilled nursing and rehabilitative care to clients in their homes.

• Conduct comprehensive head‑to‑toe assessments and identify subtle changes in patient condition.
• Triage and manage urgent clinical situations independently in the home setting.
• Perform medication reconciliation, patient teaching, and safety monitoring.
• Complete comprehensive wound assessment, care, and documentation.
• Execute acute and post‑acute care duties, including CHF, COPD, wound care, IV therapy, and post‑surgical care.

• Work independently with clear escalation procedures.
• Make real‑time clinical decisions without immediate on‑site support.
• Demonstrate strong problem‑solving skills in unpredictable home environments.
• Manage a dynamic visit schedule, prioritizing urgent visits.
• Display efficiency with point‑of‑care documentation and technology.
• Maintain attention to detail while meeting productivity expectations.
• Provide care across varied home environments.
• Show cultural sensitivity, respect for patient autonomy, and adaptability to changing patient needs.

• Current registered nursing license in good standing in Massachusetts.
• Graduation from an accredited and approved nursing program, with a school transcript or diploma.
• Minimum one year of current or recent hands‑on medical experience within the past two years with adult/geriatric populations in acute care, emergency department, home health, hospice, ICU, med‑surg, sub‑acute care, or rehab units (medical or skilled nursing settings).

• Competitive pay ranges with a weekly pay schedule; base pay $60‑$65 per point, subject to negotiation.
